The 23-year-old is a Manuel Pellegrini favourite after he nurtured the Spaniard when he was a teenager at Malaga, and is keen to be reunited with the player, claim the Daily Mirror.
Isco has managed to string a run of first-team starts together this season due to Rafa Benitez's squad being hit by injury, but has often found himself being overlooked by the likes of James Rodriguez.
The Spanish international is entering the final 24 months of his deal this summer and he is expected to open up contract talks with Real soon.
However, he is understood to be unwilling to enter into talks over a new deal unless he is assured of regular first-team football.
Isco has made 16 appearances for Real Madrid so far this season and was given a standing ovation by Madrid fans despite being sent-off in the 4-0 defeat to Barcelona.
The midfielder has also been linked with January moves to Arsenal and Chelsea after reportedly saying he wanted a 'change of scenery'.
