Corriere dello Sport claim the Brazilian will arrive in Turin imminently to complete the switch.
The same report also reveals the Italian giants will step up their pursuit of Schalke attacker Julian Draxler once a deal has been completed.
Draxler, 21, has long been linked with Arsenal but they appear to be losing ground in the race for his signature.
Officials from the German club have previously stated any potential suitor would need to table an "astronomical offer" to stand any chance of getting him.
Meanwhile, Ramires appears to be surplus to requirements at Stamford Bridge and a deal for the Chelsea man could prove far simpler.
Although boss Jose Mourinho has expressed concerns at failing to strengthen his squad significantly, the Brazilian might still be allowed to move on.
The 28-year-old joined the Blues from Benfica for a reported £15million in 2010.
In five years in England, he has made 147 Premier League appearances, playing a key role as Chelsea lifted the league title in 2015.
He has also won the FA Cup, the League Cup, the Europa League and the Champions League in his time at Stamford Bridge.
